Please submit a final report of the activities funded by the above referenced grant received from the Dekko Foundation We ask that you answer all of the following questions briefly and informatively using no more than three pages.

This report helps the Dekko Foundation better understand the programs we fund We hope that you will view it as an opportunity to share information as well as reflect upon the progress of your program Feel free to contact the Dekko Foundation’s grants manager by calling 260-347-1278 if you have questions about how to complete this report.

1. Give a brief statement of the project funded by this grant.
2. How many young people were served by your project?
3. We understand that, from the planning phase to the implementation phase, things often change (either intentionally or unintentionally). Compare the project that you proposed to the Dekko Foundation with what actually took place Were there differences? If things were changed, added or omitted, briefly explain why.
4. Discuss how your project helped to achieve the following objectives which you set for yourself:

5. Please describe any challenges that occurred as you carried out your project How did you address those challenges Were you successful in overcoming the challenges?
6. Looking at your project as a whole, what was most and least successful about it? What did you learn? What will you do differently next time?
7. Can you share an example that illustrates this grant’s impact on young people you serve?
8. Please attach a budget page which compares your proposed project costs with your actual project costs.
